Position Summary

Blended Case Manager provides services that assist children, adolescents and their families with mental illnesses or emotional disorders by gaining access to psychiatric, medical, social, educational, and other recovery oriented services. The level or type of case management service is determined by the intensity of the needs of the child, adolescent and family. Case management services include linking with services and resources, monitoring of service delivery, gaining access to services, assessment and service planning, problem resolution and help in using community resources. Blended case managers use a person-first approach and maintain a recognition that all children and adolescents have unique strengths, needs, challenges, hopes, and dreams.

The job is primarily community based with some office work and some options for work from home as appropriate.

Essential Duties and Functions

  • Assist children, adolescents, and their families in pursuing recovery oriented goals by providing services in the home and community.
  • Build relationships with key community organizations and referral sources.
  • Maintain a strong knowledge of available resources and service providers.
  • Assist individuals served in the development of goals by means of a behavioral health treatment plan that is driven by the individual's hopes, dreams and aspirations.
  • Schedule and implement visits to people in recovery.
  • Meet with clients in the community, their home, school, etc.
  • Coordinate and lead interagency team meetings as needed to support individuals in services and their families.
  • Attend school meetings as necessary in order to support and advocate for children and adolescents in services.
  • Model recovery and resiliency principles, cultural competency, and trauma awareness and incorporate them into daily interactions with participants and colleagues.
  • Assist participants in recovery in completing necessary paperwork and assessments (environmental matrix, consents and releases of information, strength and needs assessment, behavioral health treatment plan, referrals, etc.)
  • Ensure all documents are completed to meet state, county, company, and department regulations and guidelines as required.
  • Embrace and implement ICWC & CCBHC values and practices into service delivery.
  • Maintain 21 billable hours per week productivity as required by the program.
  • Complete Progress Notes and daily service logs within 24-48 hours of services rendered within the EHR.
  • Track and maintain compliance of all required paperwork and electronic documents.
  • Coordinate case conferences with Therapists, Psychiatrists, Family Members, PCP, and others as required to ensure person's supports in recovery are maintained and natural supports are put into place.
  • Participate in clinical consultations with the recovery coaching supervisor, the team, the clinical liaison, and payors.
  • Attend all scheduled trainings to help expand knowledge base of the Recovery model and other trainings as required by RHD Corporate, County, State, another licensing and funding bodies.
  • Attend and participate in weekly supervision and team meetings.
  • Attend and participate in monthly staff meetings.
  • Assist in the orientation of new staff as needed.
  • Assist with and communicate any crisis needs, as well as any other clinical information to supervisor as needed to support proper clinical care.
  • Initiate and/or participate in helping an individual access a higher level of care (ex. Inpatient hospital, partial hospital, children and youth services, etc.)
  • Other duties as required by supervisor, mobile supervisor, Operations manager, Director, or others on the leadership team.
  • Carry and answer after hours calls via an on call phone 2-3 times per year.
  • Participate in marketing and positive community interactions as needed.

Qualifications

  • Bachelor's degree in human service field or a high school diploma and 12 semester credit hours in sociology, gerontology, or other social science and 2 years experience in direct contact with mental health consumers, or a high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.
  • At least 2 years experience working with children in a social service agency is preferable.
  • 1-2 years' experience in case management is preferable.
  • Valid Driver's License
  • Strong knowledge of recovery and resiliency principles.

Working conditions

Additional Special Working Conditions:

  • Working out in community within the service delivery area of the program. Required to drive program participants to appointments.
