Responsibilities:

The RN Case Manager is directly responsible for the clinical outcomes, case management, and direct patient care of home healthcare patients. This role operates as the central hub of a multidisciplinary team, balancing compassionate patient advocacy with strict regulatory compliance.

Clinical Care & Case Management

  • Comprehensive Assessments: Evaluate physical, mental, psychological, functional, financial, and spiritual needs.

  • OASIS & Documentation: Complete required OASIS assessments accurately and ensure all clinical charts are timely, complete, and reimbursable.

  • Care Plan Execution: Formulate, implement, and continuously evaluate a home care plan in conjunction with physicians and Advanced Practice Clinicians (APCs).

  • Direct Nursing Intervention: Provide skilled hands-on nursing, updating interventions as the patient's condition or care environment changes.

Coordination & Workflow Management

  • Multidisciplinary Leadership: Coordinate care across LPNs, CNAs, community resources, and support services.

  • Oversight & Delegation: Delegate tasks appropriately to LPNs and CNAs while monitoring outcomes.

  • Caseload & Scheduling: Autonomously self-schedule patient visits to meet agency productivity standards while respecting patient preferences and communicating changes promptly.

  • On-Call & Coverage: Participate in a rotating schedule for holiday, evening call, and weekend coverage.

  • Preceptorship: Act as a professional role model, mentor, and preceptor for newly hired nurses and students.

Qualifications:

  • Associate's degree in Nursing required

  • New Mexico RN License required

  • Must have current valid driver's license and auto insurance

  • Minimum 1 year in direct patient care setting or home healthcare

  • BLS required at time of hire
