JOB DESCRIPTION Make your mark for patients We are looking for a compassionate, bilingual, and patient-focused Onward Care Coordinator to join our Rare Disease Patient Support team. This is a remote position aligned with West Coast working hours (PST) supporting Spanish- and English-speaking patients and caregivers throughout their treatment journeys. About the role As a Bilingual Onward Care Coordinator, you will serve as a primary point of contact for patients enrolled in Rare Disease Patient Support Services. You will deliver individualized, high-touch assistance focused on treatment access, affordability, administration, and adherence. Using your clinical experience and behavioral science skills, you will help patients and caregivers navigate barriers, identify relevant resources, and communicate confidently with their healthcare providers. This is a non-clinical support role, and treatment-related or medical questions will be referred to the patient's healthcare provider. Who you'll work with You will collaborate with Rare Case Managers, Rare Reimbursement Executives, access specialists, patient experience colleagues, and Rare Disease Patient Support Program operations. This positions reasonably anticipated base salary range is $88,000.00 -$115,500.00 annually. The actual salary offered will take into account related knowledge, skills, experience and location among other factors and may fall outside the expected range.
