SUMMARY

The Social Worker I provides direct psychosocial support, discharge planning, and care coordination for patients and families in collaboration with physicians, case managers, and the interdisciplinary care team. This role assesses patient needs, identifies barriers to care, develops treatment and transition plans, and connects patients with appropriate community and post-acute resources to promote optimal outcomes.

ESSENTIAL FUNCTIONS
  • Conduct psychosocial screenings and full assessments for patients requiring social work intervention.
  • Develop and implement individualized care plans focused on psychosocial support, discharge planning, and transition of care.
  • Collaborate with the interdisciplinary team to support care coordination and patient progress.
  • Provide patient and family education, counseling, and crisis intervention as needed.
  • Connect patients and families to community, legal, financial, and behavioral health resources.
  • Facilitate family meetings, referrals, and other support services, including abuse/neglect reporting, guardianship, psychiatric referrals, and end-of-life planning.
  • Maintain timely, accurate documentation in accordance with departmental and regulatory standards.
  • Participate in interdisciplinary rounds, quality improvement activities, committees, and on-call/coverage responsibilities as assigned.
  • Perform other duties as assigned.
QUALIFICATIONS

Education

  • Master of Social Work (MSW) required

Licensure

  • Maryland LMSW required

Experience

  • Up to one year of healthcare-related experience preferred
  • Relevant field placement may be considered

Skills

  • Strong interpersonal and communication skills
  • Ability to work effectively within an interdisciplinary team
  • Knowledge of community resources
  • Strong assessment, problem-solving, and documentation skills
  • Proficiency with electronic health records and standard office technology
