The PECAN Program is a comprehensive, multidisciplinary nutrition program within the Heart Center that provides individualized, highly specialized support for pediatric patients with congenital heart disease. The team is seeking a full-time nurse to support programmatic growth and care coordination for this special population.

Key Responsibilities:
  • Providing team-based care to patients with known congenital or acquired heart disease and arrhythmias, requiring cardiology and nutritional team interventions for growth and nutritional evaluation.
  • Supporting patient care in both Inpatient (Boston) and Outpatient Cardiology .
  • Attending clinic sessions to facilitate patient flow, complete nursing assessment, and provide patient and family education
  • Ensuring care coordination and timely follow-up by communicating with families, primary care providers, other specialists and community agencies
  • Providing discharge planning including patient and family education and coordination with the inpatient unit for transition to home care
  • Triaging phone calls from parents and outside providers as well as assisting with provider messages and care coordination.
  • Supporting, then transitioning to oversight of Outpatient "Cares for Kids" visits (Mass Health initiative)
  • Supporting, then transitioning to oversight of Remote Patient Monitoring within PECAN
  • Working 40 hours per week, working at Boston and Waltham satellite locations.
Minimum qualifications:
Education:
  • BSN degree
Experience:
  • At least 2 years of ambulatory or inpatient pediatric nursing experience preferred, with preference for pediatric cardiac care or GI experience.
  • Strong knowledge of NG tube placement and care
  • Strong written and verbal communication, organizational and collaboration skills
  • Preference for EPIC EMR and Prior Authorization experience
License/certification:
  • MA RN License.
  • BLS required, PALS required within 1 year.

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.

Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
