ThePACE Senior Care Manager-Prevention, Assessment, Connections, Empowerment- plays a key role in helping families stay together, strengthening kinship and natural supports, supporting permanency, and empowering youth and families through voluntary and court-involved services. Using family-centered, trauma-informed, and strength-based practices, this position leads family finding and engagement efforts, coordinates support networks, assists with placement stabilization and reunification planning, and works closely with families, staff, providers, and legal partners to create safe, lasting solutions. Work Schedule Primarily Monday through Friday, 8:00 a.m. to 4:30 p.m., with some flexibility required to meet the needs of families.
