Registered Nurse (RN) Nurse Navigator Hospital at Home HPMC

Location: Supporting patient at HealthPark Medical Center,9981 So HealthPark Circle,Fort Myers FL 33908

Department: Hospital at Home (Orion Command Center Main Office, Orion Drive, Fort Myers, FL)

Work Type: Full Time (72 hours bi-weekly)

Shift: Days, 7:00:00AM to 7:30:00PM

Minimum to Midpoint Pay Rate:$35.04 - $47.30 / hour

This position is a "hands off" RN role.

Facilitates a timely, integrated and coordinated approach to the care of acute patients in the home setting (Hospital at Home).

Uses a multidisciplinary approach and acts as a liaison between the patient, staff and acute care hospitalist as well as ancillary areas of the hospital system.

Identifies learning needs and content for educational programs.

Develops, presents, coordinates and evaluates educational activities.

Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.

Education: Bachelors degree in nursing required. Masters preferred.

Experience: Minimum of three (3) years acute care RN clinical experience.

Certification: American Heart Association Provider Basic Life Support (BLS)

License: Current RN Licensure in the State of Florida.

Other: Strong communication and interpersonal skills with the ability to flex and adapt to evolving environment and role. Computer systems, telephone/video, and customer service skills required.
