The Social Services Designee plays a vital role in ensuring residents receive compassionate, person-centered care that promotes their dignity, independence, and overall quality of life. Working collaboratively with the interdisciplinary team, the Social Services Designee assesses residents' psychosocial, emotional, and adjustment needs and develops individualized interventions that support their well-being throughout their stay. Responsibilities include conducting comprehensive psychosocial assessments, participating in care planning and resident care conferences, advocating for residents' rights and preferences, and providing emotional support to residents and their families. The Social Services Designee assists with admission, discharge, and transition planning, coordinates referrals to community resources, and helps residents and families navigate financial, legal, and social service programs when appropriate. The Social Services Designee also works to identify and address concerns related to resident adjustment, grief, loss, abuse prevention, behavioral health, and family dynamics while maintaining compliance with federal, state, and facility regulations. Through effective communication, compassionate advocacy, and collaboration with healthcare professionals, the Social Services Designee helps create a safe, supportive environment where residents can achieve their highest practicable level of physical, mental, and psychosocial well-being. To be eligible for consideration applicants should have: Required Education and Experience * High School diploma or equivalent. * Two years' experience in a social services role, preferably in a health care setting.
