Description
The Semel Institute for Neuroscience and Human Behavior is seeking a compassionate and highly organized part-time Care Coordinator to support TIES for Families, a nationally recognized interdisciplinary program dedicated to improving outcomes for children with complex needs adopted from foster care. Working closely with the Director of Clinical Services and a multidisciplinary team, you'll play a key role in coordinating care, supporting families, and ensure services are delivered seamlessly. Bachelors degree in social work, psychology, public health or related field (highly preferred)
Knowledge about providing client advocacy and liaison with community agencies
Experience in organizing and scheduling consultations and providing internal and external referrals when necessary
Minimum 3 years of experience in providing coordination of services and treatment for families involved in the foster care and option process (preferred)
Ability to develop databases, analyze reports, utilize various communication tools and search engines using hardware and software programs
Ability to communicate effectively with therapists and consultants
Skill in working as part of a team and collaborating with management, staff and supervises
Excellent interpersonal skills to interact effectively with professionals, staff, and clients
Skill in creative problem solving and being assertive but tactful in client case management in a fast paced work environment
Ability to handle multiple responsibilities and complicated issues successfully by maintaining control of the process and follow through on the relevant details
Skill in formatting and writing grammatically correct chart notes, memos, letters, and reports
Demonstrated ability to handle confidential material with discretion
Must be able to comply with departmental and program attendance standards
