About the Role

Heritage Behavioral Health Center is seeking a full-time Lead Therapist to lead our Community Support Team (CST). CST provides mental health rehabilitation services designed to decrease hospitalization and crisis episodes and to increase community functioning so individuals can achieve their rehabilitative, resiliency, and recovery goals.

As the team's designated full-time Qualified Mental Health Professional (QMHP), the Lead Therapist provides clinical leadership and direction to a multidisciplinary team delivering individualized, community-based services - in accordance with Illinois Rule 132/140 requirements.

Qualification Annual Salary Schedule
Master's Degree in counseling, social work, psychology, or related human services field $60,000 - $62,000 Every other Friday off - paid
LSW or LPC $64,000 - $66,000 Every other Friday off - paid
LCSW or LCPC $67,000 - $70,000 Every other Friday off - paid
What You'll Do
  • Serve as the full-time QMHP team lead, providing clinical oversight, supervision, and direction to team members in accordance with Illinois Rule 132/140 requirements
  • Complete and oversee mental health assessments and the development of individualized, person-centered treatment plans that reflect recovery-oriented rehabilitative goals
  • Provide and supervise the delivery of individual community support services, including illness self-management, skill building, and development of adaptive and compensatory skills
  • Support individuals in identifying and strengthening natural supports and in accessing community resources to promote independent community functioning
  • Monitor clinical outcomes with a focus on decreasing hospitalization and crisis episodes and increasing community tenure and functioning
  • Lead interdisciplinary team meetings and clinical staffings to review caseloads, treatment progress, and crisis response planning
  • Collaborate with internal and external providers - including hospitals, crisis services, residential programs, and primary care - to support continuity and transitions of care
  • Maintain accurate, timely, and thorough documentation in the electronic health record in compliance with agency, Medicaid, and regulatory standards
  • Model and support a trauma-informed, recovery-oriented, and person-centered team environment
Qualifications

Required:

  • Master's degree in counseling, social work, psychology, or a related human services field
  • Must meet the definition of a Qualified Mental Health Professional (QMHP) under Illinois Rule 132
  • A valid driver's license, reliable transportation, and proof of current automobile insurance

Strongly Preferred:

  • Licensure as LSW, LCSW, LPC, or LCPC
  • Minimum of two years of experience providing community-based behavioral health services to adults with serious mental illness
  • Prior supervisory or team leadership experience
