Job Summary
The Pharmacy Resident participates in a planned rotation schedule in accordance with the requirements set forth by the American Society of Health-System Pharmacists (ASHP) designed to foster an accurate, safe and effective provider of patient care. The Resident is responsible for assisting in the daily provision of pharmaceutical services to the patients of the organization. This includes the processes of drug procurement, dispensing, monitoring, as well as the provision of drug information to the associated professional staff and patients.
Essential Functions
  • Completes goals and corresponding outlined criteria including activities, projects and assignments for each area of rotation as assigned by Residency Director, preceptor or designee.
    Participates in all functions and assumes all responsibilities of the rotation experience as described in the rotation syllabus.
    Participates in clinical rounds as assigned by Residency Director or designee.
    Completes major projects (including a research project) as assigned by Residency Director or designee.
    Responds to drug information questions with professional, satisfactory, and timely replies.
    Keeps up-to-date on the latest developments concerning new drugs or developments reported from research organizations regarding drug usage.
    Comprehensively reviews and presents material at assigned professional and community lectures, in-services, continuing medical education and journal clubs.
    Supports resident recruitment at pharmacy conferences and helps coordinate the interviewing process.
    Reviews and complies with all guidelines as described in the residency manual.
    Serves as a co-preceptor for pharmacy students, meets the preceptor requirements of the relevant College(s) of Pharmacy, and actively participates in the education and training of new pharmacy employees and pharmacy interns.
  • AHSP residency goals include pharmacy practice management responsibilities; these are achieved by performing the job functions of a hospital pharmacist.
    Assures the accurate and timely dispensing and distribution of all pharmaceuticals.
    Dispenses medications promptly to meet the needs of the patient.
    Interprets and clarify orders for all types of medications.
    Accurately read, extract and interpret information in patient charts.
    Accurately complete controlled substance documentation as required by law.
    Accurately maintain investigational drug records and documentation as required by study sponsors and accrediting bodies.
    Demonstrates appropriate use of aseptic technique.
    Verifies and monitors the preparation of sterile and non-sterile compounded products.
    Supervises and directs pharmacy support personnel, including the daily activities of technicians.
    Demonstrates proficiency in chemotherapy preparation and order processing.
    Accurately re-establish priorities based on workload and patient needs and will ensure that medication doses are delivered within established departmental guidelines.
    Monitors drug therapy as ordered by the medical staff and suggests alternative drug therapy when appropriate.
    Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions, and appropriateness of drug and dose.
    Documents clinical activity/interventions as performed.
    Provides pharmacokinetic consultation services as directed per policy.
    Monitors and adjusts medications based on patient specific factors (renal function, weight, etc.) as directed per hospital protocol.
    Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.
    Reviews medication orders for appropriate IV to PO interchanges as directed per hospital protocol.
    Evaluates need for all non-formulary medication requests and initiate approved therapeutic medication substitutions when indicated.
    Performs total parenteral nutrition (TPN) monitoring and documentation per pharmacy guidelines and communicate therapeutic recommendations as indicated.
    Acts as a consultant to healthcare professionals and patients regarding utilization of pharmaceutical therapies.
    Promptly responds to inquiries with evidence-based and cost-effective therapy recommendations.
    Provides direct patient interaction via bedside encounters, medication reconciliation, discharge counseling or as otherwise directed.
    Promptly performs patient assessments and participate in the multidisciplinary clinical rounds on the patient care unit as directed.
    Participates in Network quality improvement initiatives as directed.
  • Performs other duties as assigned.
Education
  • PharmD from an ACPE-accredited School of Pharmacy - Preferred
  • Bachelor's Degree Pharmacy from an ACPE-accredited School of Pharmacy - Required
Experience
Licenses and Certifications
  • Clinical Other\ACLS - Advanced Cardiac Life Support - Preferred
  • Pharmacy\RPH - Registered Pharmacist Exam eligible or Current and valid State of Arizona Pharmacy (RPh) license. - Required
