Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary: Job Responsibilities: 1)Work with Program Coordinator and school staff to schedule workshops in high schools at the FHC school-based health center and other schools in CBOs in Brooklyn. 2. Conduct evidence-based sexual health workshops using Making Proud Choices curriculum.. 3. Administer sexual health risk assessment surveys to all workshop participants 4. Meet individually with workshop participants identified as high risk to provide risk reduction counseling connect them to the FHC School Based Health Center or other social services. 5. Promote FHC School Based Health Centers services 6. Provide sexual health education to youth at FHC School Based Health Centers 7. Assist patients in enrolling the Family Planning Benefit Program as needed. 8. Provide rapid HIV counseling at FHC School Based Health Centers and complete all related documentation. 9. Participate in project SAFE community events to connect youth STI and HIV testing. 10. Participate weekly staff meetings at Project Reach Youth 11. Submit workshop attendance information in ETO Minimum Qualifications: Preferred Qualifications: Qualified candidates must be able to effectively communicate with all levels of the organization. NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $49,200.60 - $51,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
