Discharge Planning Coordinator

Position Summary:

The Discharge Planner is responsible for coordinating the movement of patients between the hospital and continued treatment after discharge. They are responsible for developing and implementing discharge plans based on the patient's needs.

Job Requirements:

• Education: Bachelor's degree in Psychology, Counseling, Social Work or related field preferred
• Current CPR/First Aid certification
• Preferred one year of experience working in a psychiatric inpatients.
