Description

Responsible for coordinating hospice services for clients, including scheduling visits according to the established plan of care, maintaining relationships with referral sources, community partners, as well as clients and their representatives, ensuring regulatory and contractual compliance, and supporting efficient clinical operations.

Position: Team Coordinator for Hospice

Hourly: $21.00 - $24.00 hourly, depending on experience

Service Area: Lifespark Corporate Office, St. Louis Park MN 55416

Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm (every other weekend rotation required). Hybrid/remote schedule is an option after the in-office training process is completed.

Position Functions & Responsibilities:



  • Coordinates hospice services for clients and supports development, implementation, and ongoing maintenance of the client's plan of care in collaboration with physicians, case managers, and the interdisciplinary team.
  • Collaborates with clinical managers and field clinicians to organize schedules, adjust assignments, and proactively plan for client care needs.
  • Schedules client visits in alignment with the ordered Hospice plan of care and discipline frequency, ensuring timely service delivery and continuity of care.
  • Will establish workflows and practices to enhance the efficiency of client assignments and work with the hospice leadership team to prepare as far in advance as possible for client care needs related to clinical assignments.
  • Audits plans of care and discipline frequency orders for regulatory, contractual, and organizational compliance, including Minnesota hospice practice standards and Hospice requirements.
  • Communicates client status updates and care summaries to Hospice case managers, interdisciplinary team, and payer representatives.
  • Maintains working knowledge of Hospice contracts and ensures services are delivered in accordance with payer provisions.
  • Works with billing staff to ensure accurate and compliant Hospice billing.
  • Answers incoming calls, triages requests, and responds promptly to client, family, facility, and payer inquiries.
  • Maintains positive relationships with referral sources, payers, and community partners, promoting high-quality hospice services.
  • Supports workflow efficiency through documentation review, data tracking, and administrative coordination.
  • Maintains current knowledge of community resources and assists with care coordination when services fall outside hospice scope.
  • Performs other duties as assigned.



Qualifications:



  • Minimum three (3) years' experience in healthcare coordination, hospice or managed care preferred.
  • High school diploma or equivalent required; Associate degree preferred.
  • Demonstrated knowledge of hospice operations, managed care processes, and regulatory compliance expectations.
