Summary:

The role involves performing nursing procedures and patient care tasks as assigned by the facility under the direction of the managing team, Director of Nurses, and/or Physician. A skilled nurse performs a variety of duties to care for and comfort patients requiring medical services. This position requires utilizing specialized skills and judgment when delivering care to patients in a long-term care setting. The role implements and evaluates patient care while performing nursing procedures to treat and rehabilitate patients.



  • Provide professional nursing services, including nursing interventions and their documentation, case finding, health teaching/counseling, and provision of supportive and/or restorative care.
  • Participate in staff training, prepare clinical and administrative reports, maintain infection control standards, and function as a member of a treatment team. You may also act as a charge nurse on a unit or in a residence, or as house or unit supervisor in a nursing home.
  • Assure that the safety and physical comfort of the patient meet as well as the patient's emotional and spiritual needs.
  • Perform nursing procedures for which training has provided the necessary skills and judgment.
  • Assist with the rehabilitation of patients according to the care plan directed by the physician.
  • Observe and record all pertinent information and report to the appropriate supervisor and physician.
  • Medication management.
  • Complete documentation as required and in accordance with the facilities' policy.
  • Accurately report patient status to supervisor.
  • Consistently practice standard precautions.
  • Perform all duties promptly in a competent and caring manner.
  • Maintain good communication among patients/residents and all healthcare personnel.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to operate and handle many kinds of medical equipment.
  • Frequently move/lift between 50-100 pounds, depending on assignment requirements.
  • Stand for long periods with frequent squatting, twisting, bending, kneeling, and reaching to prepare equipment, materials, or objects in order to provide patient care or maintain the patient environment.
  • Comply with all facilities' policies, procedures, and practices.
  • Assist with transfers and ambulation.


Requirements:

  • Currently a Licensed Practical Nurse in New York State.
  • Preferred Long Term Care/Skilled Nursing experience.
