The Licensed Practical Nurse performs routine nursing care of the ill, injured, or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner, and/or a physician. The LPN participates in the planning, implementation, and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues that affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in the policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate if needed to other Care Network sites within the designated cluster. Provides and documents direct nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention, and modification of the plan of care. Assists other health care personnel in the delivery of patient care. Participates in maintaining the environment of care including equipment and other material resources. Participates in own professional development by maintaining required competencies and attending educational offerings. Supports the development of other staff and formal learners. Participates in the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities. The LPN may administer medications that are within their scope of practice and organizational standards only if the following conditions are met:
Supports clinical activities related to nursing and clinic flow and makes recommendations to the manager for improvement in the quality and effectiveness of nursing care given. Demonstrates commitment to patient safety by speaking up about safety issues and changing nursing practices to enhance safety. Shares errors through system reporting mechanisms and demonstrates shared accountability for safe practices. Performs other related duties incidental to the work described herein.
