Registered Nurse (RN) Patient Care Navigator NICU

Inova Fairfax Hospital NICU at Inova L.J Children's Hospital is looking for a Full Time RN Patient Care Navigator to work M-F 8-4

The NICU Patient Care Navigator serves as a key liaison between families and clinical teams ensuring seamless communication and coordinated care. This role leads careplanning meetings, milestone conferences, and weekly interdisciplinary rounds, while also managing updates related to changes in care and discharge planning. The Liaison partners with state agencies and collaborates with community organizations. In addition, this position provides essential family education-including CPR instruction and preparation for transition home-and trains staff to effectively support and educate families.

Inova L. J. Murphy's Children's Hospital is home to the largest, most comprehensive subspecialty neonatal intensive care unit (NICU) in the region. Our 108-bed, Level IV NICU cares for high-risk infants requiring specialized care and observation. The NICU is also the first in the nation to receive The Joint Commission's Gold Seal of Approval for Prematurity. This certification represents the highest level of care for premature birth patients.

RN Patient Navigator Responsibilities

• Guides patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care.
• Acts as a liaison, advocate and point of contact to patient by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services.
• May assist patients and families with resolving psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources.
• Acts as advisor/educator by providing emotional support, clinical education, resources and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
• Builds working relationships with patient, healthcare providers, and community resources to maximize patient outcome and identify/resolve barriers to the plan of care.

RN Patient Navigator Qualifications

• Minimum of three (3) years' experience in nursing
• BSN Required
• Current Basic Life Support (BLS) from AHA
• Minimum of three (3) years' experience in NICU

RN Patient Navigator Preferred Qualifications:

• Minimum of three (3) years' experience in NICU
