Description
Summary: A professional who utilizes the nursing process to assess, reassess, implement, coordinate, and evaluate patient care activities, under the supervision of a Registered Nurse. Collaborate with other Clinical Associates and healthcare disciplines to meet individual needs using appropriate resources to ensure safe, quality, cost-effective care. Nurtures a relationship with physicians, nursing staff, and other disciplines involved in the care of the patient/family. Responsibilities:
- Under the direction/supervision of an RN, effectively manages patient care delivery as assigned:
- Completes and documents reassessment of patient needs according to department standards.
- Documents and communicates an appropriate multidisciplinary plan of care based on identified problems and/or patient needs.
- Collaborate with patient/family/physician and other disciplines to coordinate the plan of care.
- Evaluates patient response to care, revises plan of care as necessary, and documents appropriately
- Administers and documents medications and treatments safely following Medication Administration policy and procedure.
- Recognizes change or urgent situations in patient condition and intervenes appropriately.
- Provide patient and/or family with information relative to treatment plan, diagnostic/surgical procedures, disease process, and care after discharge. Initiates collaboration and documents appropriate teaching, considering age, cultural and religious beliefs, and barriers to learning. Evaluates the effectiveness of teaching and provides appropriate follow-up.
- Performs Point of Care Testing within the scope of licensure; accurately performs testing according to the approved procedure and limits testing to only those tests for which competency has been assessed, and the operator is deemed qualified by a designated trainer.
Job Requirements: Education/Skills
- Graduate from an accredited school of practical nursing or vocational nursing
Experience
- One year as hospital staff level LPN in assigned area
Licenses, Registrations, or Certifications
- Current Louisiana LPN license.
- BLS provider certification required
