The Role

As a Clinical Pharmacist supporting Medicine and Cardiology services during evening shifts, you will be a key clinical resource for providers, nurses, and patients across the inpatient setting. Your work directly supports safe, evidence based medication use and continuity of care during a pivotal time of day.

In this role, you will

• Provide comprehensive clinical pharmacy services to adult medicine and cardiology patients in an acute care academic setting

• Participate in interdisciplinary collaboration with physicians, advanced practice providers, nursing, and care coordination teams

• Optimize pharmacotherapy for complex cardiovascular and medical conditions, including high risk medication regimens

• Review and verify medication orders with a strong focus on safety, efficacy, and guideline directed therapy

• Serve as a clinical consultant during evening hours, supporting therapeutic decision making and urgent clinical needs

• Contribute to transitions of care, medication reconciliation, and patient specific education as appropriate
