In this role you will be responsible for direct patient care and case management of Hospice patients in outpatient settings. Ensures that end-of-life care is delivered within quality and utilization guidelines and is directed toward the achievement of desired clinical outcomes for patients and their families. Responsible for development, coordination and scheduling delivery of care through a multidisciplinary team to the patient in the home or community setting. The RN Case Manager assures completeness, timeliness and appropriateness of the patient care plan, its implementation, and ongoing evaluation. Assures communication with the multidisciplinary team, support services and community resources to ensure that plan of care goals are achieved in the most efficient and cost-effective manner possible. In all care, utilizes nursing assessment skills to determine presence of symptoms and response to interventions for those symptoms. Assessments include a review of the full spectrum of patient needs, including an understanding of how the physical, psychological/social (behavioral), functional, financial, resource and spiritual needs of patients factor into their condition. Serves as a consultant to discuss goals of care with the patient/family and communicate those goals to the appropriate care team. In addition, reviews referrals, and delegates the referral to the appropriate hospice/palliative care staff based on patient needs and staff availability. Performs Hospice consult reviews and coordinates a clinical assessment, determines if patient is appropriate for hospice, communicates with the patient/family about hospice philosophy, criteria, and site of care. Assumes role of hospice RN with hospital hospice patient, including development of hospice plan of care and subsequent rounding, if necessary. Acts as a clinical resource for all members of the hospice /palliative team. Develops and maintains a working knowledge of all services available within the Home and Transitions Service Line. Completes comprehensive and accurate documentation timely and consistently that clearly reflects interventions performed. In all roles, performs as a member of an interdisciplinary team. Assists with orientation and mentoring of new team members as appropriate. Serves as a resource person & professional role model. Performs other functions as required.
