We seek an experienced, enthusiastic Physical Therapist who thrives in an autonomous setting and values collaboration with a dynamic team of therapists. Help clients apply skills in real-life environments, such as kitchens, grocery stores, restaurants, and outdoor spaces.

Opportunity for increased responsibility as a Clinical Coordinator!

Perform patient evaluations and develop personalized treatment plans.
• Regularly reassess treatment effectiveness and participate in staffing meetings and family conferences.
• Conduct discharge evaluations, recommend home and community safety modifications, and provide education and referrals.
• Communicate patient progress and needs to the treatment team, physicians, patients, and families.
• Collaborate with other disciplines to coordinate care and enhance patient outcomes.

• Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in the practicing state.
• CPR or Basic Life Support certification per state regulations.
• At least one year of physical therapy experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injuries.
• Strong verbal and written communication skills.
• Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
• Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.
• Health screenings or exams may be required per location or state policies.
