Candidate needs to live within commuting distance of the Allina Commons Minneapolis, MN

  • 1.0 FTE (80-hours per two week pay period)
  • 8-hour day shifts
  • Occasional weekends

Job Description:

Provides clerical and support functions to the social work and case management staff members in the areas of clinical information, transportation, durable medical equipment (MDE)/oxygen needs, etc. with the goal of promoting expeditious patient transfers and/or discharges. This is accomplished through the major job components of coordination/communication, delegated responsibilities, role modeling, quality/resource stewardship and patient advocacy/customer service.

Principle Responsibilities

  • Lead Duties
    • Assists manager in providing direction and guidance to care team.
    • Assists with audits, workflow standardization, process implementation and policy/procedure content.
    • Drives the team to meet key performance indicators and monitors program standardization.
    • Assists in improving time management to increase teams productivity.
    • Participates in recruitment process for new hires, including orientation preparation, scheduling shadow visits, as well as follow-up on mandatory education.
  • Clerical Service Coordination
    • Supports patient transfer and discharge efficiently and effectively, prioritizing post acute discharges including, but not limited to, skilled nursing homes and home health care.
    • Assists with preparing the required patient medical record information for discharge to nursing homes, home care hospice or other medical facility.
    • Sends protected health information accurately via fax, telephone, hard copy, etc.
    • Contacts nursing homes to determine bed availability and communicates information to department staff.
    • Assists with arranging transportation as requested.
    • Assists with medication authorization, taxi vouchers etc. as requested by department staff.
    • Communicates and exchanges information effectively and timely to appropriate personnel, department or outside agency to promote continued progression of plans of care and discharge.
    • Prioritizes and implements department and patient needs according to unit/hospital policies, procedures and standards.
    • Assists with data collection and reporting projects as assigned.
  • Patient Advocacy/Customer Service
    • Provides assistance to the department when Admin Assistant is absent i.e. phone and reception duties, sorting of mail etc.
    • Partners with other Care Management Department staff members to promote timely discharges.
    • Responds timely to requests for assistance in a timely, efficient and positive way.
    • Maintains confidentiality of patients, families, and other staff at all times.
    • Communicates with staff and other key stakeholders in timely, understandable and responsive manner to meet needs and questions.
    • Establishes positive working relationships with community resources.
  • Quality Resource/Stewardship
    • Utilizes resources effectively and efficiently.
    • Participates in quality improvement activities to evaluate and improve systems and processes that support patient care.
    • Effectively utilizes time and applies organizational skills to achieve productivity and promote efficient utilization of resources.
    • Utilizes supplies and equipment cost effectively and demonstrates quality care
    • Flexibility and creativity is demonstrated in response to meeting the changing patient, Department and staff needs.
    • Identifies and communicates quality concerns/suggestions for positive change and incorporates changes based on quality improvement results into work
    • Demonstrates safe work practices to prevent injury to self, peers and/or patients.
  • Other duties as assigned.

Required Qualifications

  • Must be 18 years of age with education and/or experience needed to meet required principle competencies as listed on the job description
  • 1 year experience in a medical setting, including medical terminology experience. Proficient in computer and clerical skills, with the ability to navigate clinical information systems and databases

Preferred Qualifications

  • High school diploma or GED
  • Associates or Vocational degree

Pay Range

Pay Range: $21.55 to $29.35 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
