Location: Orlando and Surrounding Areas, FL

Position Summary

The Physical Therapist (PT) provides medically prescribed, goal-oriented therapy in accordance with the Physician's Plan of Care, company policies, and professional standards. This position requires collaboration with the interdisciplinary team to deliver high-quality, patient-centered care in the home setting.

Key Responsibilities

Develop and implement individualized physical therapy plans of care.

Perform in-home visits to evaluate, treat, and reassess patient needs.

Communicate regularly with physicians, care coordinators, and interdisciplinary team members.

Educate patients and families on home exercises, safety, and wellness strategies.

Participate in interdisciplinary team meetings to review and coordinate care plans.

Maintain accurate and timely documentation within the EMR system.

Comply with all agency policies, ACHC standards, and federal/state regulations.

Qualifications

Current Florida PT license in good standing.

Specialized training and experience in pelvic floor therapy required.

Knowledge of physical therapy principles, concepts, and methodologies.

Excellent communication, critical thinking, and problem-solving skills.

Ability to work effectively in a collaborative healthcare environment.

Required Credentials

Current CPR certification

Level 2 Background Screening

Recent negative TB test or chest X-ray
