LGBTQ Health - Social Worker II We're looking for those individuals-the creative thinkers and innovation seekers-who are content with nothing short of changing the world. In the role of Social Worker II you will provide clinic-based medical social work consultation services to the LGBTQ+ community of patients receiving their care in the Inclusion Health Clinic at Froedtert Hospital. Medical social work services may include care coordination, benefits access and navigation, health literacy support, health system navigation and advocacy, referral, and community care coordination. To participate as a member of a multidisciplinary care team providing support to members across the LGBTQ+ community. Primary Functions
Knowledge - Skills - Abilities Knowledge of medical systems, health care setting and LGBTQ+ specific health care needs. Understanding of insurance benefits available for patients across social service systems. Ability to provide mental health screening and appropriate referral, as well as crisis triage and management. Ability to function as an interdisciplinary team member. Sociocultural sensitivity. Ability to respond to clinical and administrative emergencies. Education and/or experience in outpatient interventions in community social service and mental health settings. Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting. Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision making skills.
