About Rehab and Nursing Center-Leffingwell The Rehab and Nursing Center - Leffingwell was designed with healing in mind. The center offers 80 private rooms with beautiful nature views. Our expert team of nurses and therapists use the latest technology and techniques to maximize independence and mobility to help prepare patients to return home. We provide comprehensive care to patients recovering from serious injuries, surgeries, or illnesses. We help patients regain their independence, improve their quality of life, and return safely to the community. Scope of Work
Provides consultative, facilitative, technical and education support to patients, families, physicians, care managers, finance departments and health care facilities with regards to post-acute insurance benefits and coverage and the types, appropriateness, levels-of-care and availability of local post-acute services in the service area. Provides formal and informal education, formal and informal consultative support, facilitates benefit verification and, assists customer groups in the placement of patients in the appropriate post acute setting. Serves as a resource to the Spectrum Health Hospital Group, Spectrum Health Medical Group, Priority Health, Medical Staff, post-acute providers, vendors and the larger community. The consultant may provide coverage for, and be deployed in support of, Post Acute Community Liaisons.
