- Ensure the social, physical, psychological, and emotional well-being of youth participating in the Rapid Rehousing Program. 
 
 - Prepare and maintain case plans on a quarterly basis. 
 
 - Conduct interviews with parents/guardians and youth to complete assessments and develop individualized treatment and service plans, including case planning, referrals, ongoing monitoring, and service coordination. 
 
 - Manage a caseload to provide appropriate resources and assist youth in navigating community services. 
 
 - Support youth in accessing medical and mental health services, medication management, activities of daily living (ADL) assistance, and AODA (alcohol and other drug abuse) services and supports. 
 
 - Communicate and report youth concerns to appropriate parties in a timely and professional manner. 
 
 - Collaborate with clients on discharge planning beginning at the time of program admission. 
 
 - Facilitate client transfers when services are no longer appropriate or when additional services are required; identify children who may qualify for Wraparound Services. 
 
 - Coordinate transportation for youth to and from appointments, including medical, mental health, and housing-related visits. 
 
 - Attend and actively participate in departmental meetings and collaborative meetings with partner service agencies. 
 
 - Document all case management activities in Clarity, the Homeless Management Information System (HMIS). 
 
 - Serve as the primary contact for landlords, maintaining effective relationships and facilitating unit turnover and client placement as needed. 
 
 - Coordinate and oversee client move-outs, including meeting with clients to discuss move-out plans and expectations. 
 
 - Assist in organizing social, recreational, and educational activities for program participants. 
 
 - Complete special projects as assigned. 
 
 - Maintain professional and technical competency by staying informed on industry trends and best practices. 
 
 - Complete and maintain all required documentation, records, and reports accurately and timely. 
 
 - Comply with all safety policies and work rules, maintaining a clean and orderly work environment. 
 
 - Wear appropriate personal protective equipment (PPE) as required (e.g., masks, gloves).
