 Be the Best Hospice Nurse Practitioner You Can Be   If you meet these qualifications, we would love to meet you:  
 Graduated from an approved school of professional nursing - Masters degree Current and unecumbered advanced practice nurse license in the state(s) practicing 
 One year of experience as an NP  
 Certified by a nationally recognized advanced nurse practice organization  
 Possess and maintain valid CPR certification while employed in a clinical role (preferred)  
 Must be a licensed driver who can travel to all business locations  
 Meet the regulations and requirements of the state(s) in which the program provides services  Responsibilities:   As a Hospice Nurse Practitioner, you will:  
 Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/familyMonitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching 
 Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs Prescribe medication as permitted by the state and/or the state Board of Nursing Pre-Hospice 
 Face-to-face clinical and needs assesment at intake, preferably in the patient's home setting, inclusive of patient's medical, emotional, spiritual, social, and practical needsFace-to-face reasssessment done when needed and at least every 60 days. Plan of care development based on these assessments and incorporting the patient's and family's prioritiesInitiate communication with attending physcians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/familyClinical supervision of the care plan including all medicine ordersCollaborate with the rest of the pre-hospice team including alerting the team to changes and newly identified patient needs as they arise Our Investment in You   Caring for others starts with caring for you.
