THE POSITION

Westmoreland County Area Agency on Aging (AAA) is seeking a professional full-time Aging Care Manager 2 to help us make a positive impact on families and the community. DESCRIPTION OF WORK

Within this important role, the Aging Care Manager 2 maintains a caseload of vulnerable consumers with the key responsibility of completing a functional eligibility determination (FED) to determine the appropriate level of care for the individual. Level of care can include nursing facilities, personal care and DOM care placement, as well as home and community-based services. Your proven interview skills will be essential as you obtain accurate information from the consumer, members of the consumers' families, neighbors and other persons necessary for the identification of social, economic, emotional, health or physical problems affecting the consumer. You will be expected to protect the security of personal health information obtained during the interview process. This position displays initiative, independent judgement, seeks advice when needed for unusual or difficult cases and is knowledgeable of both state and federal regulations as they apply to the agency.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. May be required to be on a standby rotation for off-hours coverage.
  • This position combines office and fieldwork.
  • Telework: After the initial training and probationary period, telework may be considered on a case-by-case basis. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

Other Requirements:
