DESCRIPTION OF WORK

In this important role, you will assist those who are returning to the community after mental health hospitalization or prevent others at risk of premature hospitalization. This work supports the consumer's efforts to reintegrate into or remain in the community, thus fostering success in recovery and in building the confidence and self-reliance needed for full independent living in the community. You will work in accordance with each consumer's Individual Service Plan (ISP) to implement specific goals and objectives, while also training them in home safety protocols. It will be essential to document observations, behaviors, and progress in a timely manner and to communicate relevant information with your supervisor and multidisciplinary team members. Additionally, you may be required to transport consumers to appointments using a county vehicle when necessary. This position requires close contact with the consumer in the home environment. Workers in this capacity will work on shifts to assure 24-hour availability in a small facility in which a limited number of seriously mentally ill (SMI) consumers reside, each in their own apartment. Work Schedule and Additional Information:
- This is part-time employment, 15-20 hours per week
- Work hours are 8:00 AM to 4:00 PM, Wednesday, Thursday, and Saturday, with a 30-minute lunch
- Often, irregular hours, including: evenings, holidays and weekends may be required
- Employee may have occasional overnight or out of county travel for meetings or training.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months of experience as a Residential Program Trainee; or
- One year of experience in a Human Services Agency working with the mentally disabled, the elderly, or children and youth; or
- Two years of college which includes courses appropriate to the Human Services field; or
- Any equivalent combination of experience and training.
Other Requirements:

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
