Bio: The Diabetes Care and Education Specialist for Maine Medical Center (MMC) provides direct patient education for persons with diabetes and their families to prepare them for safe diabetes self-care after discharge. This consulting service receives referrals from providers and nurses throughout MMC. Additional responsibilities include acting on outcomes data, assisting with clinical diabetes management protocols or pathway development, educating healthcare professionals, and acting as a healthcare resource for MMC and MaineHealth. The Diabetes Nurse Care and Education Specialist will collaborate his/her work efforts with the inpatient units and associated care providers as well as the multidisciplinary inpatient glucose control project management team. Summary: Diabetes Nurse Educator's main role is as a diabetes education specialist for Maine Medical C enter. Additional responsibilities include acting on outcomes data, assisting with clinical diabetes management protocols or pathway development, and healthcare resource. Incumbent will be a member of the outcomes research team for the project. The Diabetes Nurse Educator will collaborate his/her work efforts with the inpatient units and associated care providers and the multidisciplinary inpatient glucose control project management team.
