Job Description

Elwyn seeks a Licensed Clinician to function as an Outpatient Therapist (OT) - PRN, under the direction and clinical supervision of the Outpatient Director, is responsible for providing assessment and therapy services for adults. The therapist is responsible for functions of coordinating, managing, supporting and facilitating appropriate services to meet the needs of individual clients.

ESSENTIAL FUNCTIONS:

  • Develop initial assessments of individual treatment needs by way of one-on-one sessions and use of any records that are sent by referral agencies
  • In collaboration with other resources-including family members, agency representatives, other agencies involved in the individual's treatment-using behavioral health assessment tools and following guidelines established by regulatory and practice requirements, develop treatment plan goals for assigned individuals
  • Maintain progress notes and progress reports, and modify treatment plans according to changes in client status
  • Provide clinically appropriate treatment, including individual and group counseling, consistent with the treatment plan goals
  • Utilize individuals' natural and professional supports as relevant to assist individuals in meeting their treatment plan goals
  • Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support
  • Maintain chart documentation according to regulatory and payer requirements
  • Ensure that all required paperwork is completed to standards prior to inspections by payer or regulatory agencies
  • Interface with other Elwyn clinical personnel, including therapists, psychologists, psychiatrists, and Certified Peer Specialists relative to the individual's overall treatment plan, in support of the overall outpatient program function and in support of sister programs (e.g. BCM, CPS)
  • Attend internal team meetings and other outside meetings as directed by the Director or designee with the goal to support the clients needs in reaching their treatment plan goals
  • Perform other duties as assigned

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

  • Master's degree required from an accredited university in a clinical mental health discipline, such as psychology, counseling, clinical social work, psychiatric nursing, or marriage and family therapy
  • Demonstrated ability to work effectively as part of a team
  • Must be able to perform CPR, first aid and crisis intervention using agency trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated, intermediate experience with Microsoft Office applications, including Word, Excel, and Outlook
  • Must be able to utilize Electronic Health Record (EHR) / Electronic Medical Record (EMR)
