Job Description:
A Care Manager is an entry level position within the field of social work, typically filled by individuals who have recently completed their bachelor's degree in social work (BSW) or master's in social work (MSW) who do not have current licensure. Under general supervision, a Care Manager will provide essential support and guidance to patients and their families as they navigate overall wellness in the OU healthcare system. The Care Manager works with the interdisciplinary care team to coordinate patients' care across the continuum of care and provide access to adequate resources. Essential Responsibilities Responsibilities listed in this section are core to the position. Assess assigned patients' unique conditions by combining data from medical diagnoses, chart reviews, personal interviews, and care teams. Assist in developing care plans for patients and their families by gathering essential information, conducting screenings, and coordinating referrals to community resources for ongoing support. Provides social services to patients and their families for an assigned medical service Performs psychosocial screening and make referrals to necessary clinical staff Provides patients and family education regarding diseases, treatments, social or financial situations, emotional stress and/or other problems Monitors care to evaluate progress towards goals and institutes appropriate interventions as needed Provides resources to patients and families regarding social determinant needs. Acts as a liaison between patients/families and the clinical staff and other agencies Advocate for patient concerns and escalate ethical dilemmas through the appropriate channels. Maintain patient confidentiality of all communications and documents as required by hospital policy and as regulated by HIPPA Performs other duties as assigned. Completes applicable reports bases on mandated reporting requirements.

Basic knowledge of healthcare regulations, including CMS guideline Strong ethical standards and professional integrity; knowledge of Code of Ethics
