The Disease Prevention & Control Program Coordinator is responsible for planning, implementation and evaluation of processes for the public health disease prevention and control program. This position works as part of a team concerned with disease investigations and immunization services and collaborates with the DPC Public Health Supervisor.

Minimum Qualifications
-
* Bachelor's degree in Nursing AND * Minimum of three (3) years of professional nursing experience in public health, community- based settings, home care, hospital or nursing facility. AND * Registration as a Public Health Nurse in Minnesota AND * A valid driver's license
PREFERRED QUALIFICATIONS BEYOND MINIMUM REQUIRED: * Master's degree Public Health, Public Administration, Nursing, or a related field * Previous experience in disease prevention and control, epidemiology, emergency preparedness and public health project planning, development and management highly desirable * Previous experience with community immunization clinics * Experience in the management and coordination of public health grant deliverables desirable

Duties & Responsibilities

These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position's management approved work schedule is required for all positions.
- Lead the assessment of the local surveillance and reporting systems to respond to infectious disease outbreaks.
- Provide overall coordination and management of the public health planning process to implement infectious disease policies and programs and grant deliverables to achieve program objectives and outcomes.
- Implement disease reporting protocols as outlined in the Minnesota Infectious Disease Operations Guide and lead the review of Dakota County Public Health Disease Prevention and Control (DPC) policies and procedures.
- Coordinate disease investigation response.
- Take lead in outreach, education and disease reporting with medical providers, schools, business and others as identified.
- Lead training and orientation efforts for Disease Prevention & Control staff, public health staff or Medical Reserve Corps participating in DPC related activities.
- Lead the development of new DPC initiatives as applicable and as capacity allows.
- Provide consultation to local, regional and statewide partners and serve as a resource for disease prevention and control.
- Collect and analyze program data, prepare local, regional and statewide program reports, plans and grant applications that support grant deliverables, program objectives and overall department needs.
- Develop, coordinate and conduct program evaluation and provide future programming strategies.
- Coordinate and train staff on various electronic systems such as HAN, PH-DOC and databases including MEDSS and MIIC.
- Participate on local, regional and statewide committees representing Dakota County.
- Coordinate and support work between Disease Prevention & Control and Emergency Preparedness work units to increase response capacity as work demands fluctuate.

WORK ENVIRONMENT: Work is done primary at the Western Service Center in Apple Valley with some opportunity to telework. There is some walking and some travel to meetings and conferences away from the regular place of work. May require some evening hours. Items carried typically include light objects such as briefcases, notebooks, work papers, and data printouts. Lifting of moderately heavy objects generally is not required. No special physical effort or ability is required to perform the work.
