JOB DESCRIPTION:

SUMMARY OF POSITION

The Liaison program works with Centerstone clients, both new and existing, who are discharging or who have recently discharged from a psychiatric hospital. The Liaison will be working as an extension of the mobile crisis team during regular hours. The goal is to ensure a smooth transition from inpatient to outpatient care. This is achieved by the Liaison team working in close connection with the client, the staff of the discharging hospital, and with the client's treatment team at Centerstone. The liaison will work to provide release planning for patients who suffer from mental health illness and/or substance abuse disorders. The liaison will ensure clients make connections to behavioral, medical health and/or substance abuse treatment.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Meet with clients to introduce Centerstone services, working with Inpatient or Emergency Departments clients who are being dischargedand being referred to Centerstone's mental health services to assist patient with completing Centerstone Registration, ROIs for alternate contacts, including the discharging hospital.
  • Coordinate a schedule with the hospital to meet with hospital staff and Centerstone clients/potential clients.
  • Create and maintain relationships with hospital staff and assisting with discharges as needed.
  • Conduct phone calls to remind clients of appointments and complete follow up calls to clients when initial post discharge appointment is missed.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Skilled in use of technology.
  • Must have a working knowledge of the targeted population and commitment to the principles of liaison services.
  • Must be knowledgeable of the services that Centerstone provides.
  • Must value cultural differences and be able to work as a team member.

QUALIFICATIONS

Education Level

Bachelor Degree in psychology, social work or related field preferred.

Years of Experience

Two years of experience working with individuals who experience severe mental illness and substance use disorders preferred.

Certification/Licensure

OQMHP required

PHYSICAL REQUIREMENTS

Pay Range:

$18.20--$25.50
