Provides leadership to multiple disciplinary team and/or an entity-wide function of Advanced Practice Providers.
Essential Functions
-Provides leadership and clinical direction of a program and/or multiple related teams and/or entity-wide function of Advanced Practice Providers (APPs).
-Serves as a strategic liaison between physicians, APPs, and other clinical staff.
-Active in entity wide strategic planning efforts and major initiatives as it relates to APPs.
-Develops operational and strategic plans for departments.
-Responsible for hiring, discipline, performance coaching, appraisals, and other APPs. -Responsible for orienting, training, APPs.
-Leads continuing education plans for APPs.
-Develops, evaluates, and manages quality and safety outcomes in collaboration with hospital and enterprise quality and safety standards.
- May performs clinical duties of a Nurse Practitioner as needed.

Master's Degree Nursing required

Nurse Practitioner [NP] / Advanced Practice Registered Nurse [APRN] [State License] - Generic - HR Only required

Experience
Nurse Practitioner Experience 5-7 years required and Clinical leadership experience 5-7 years required, and Clinical management experience 3-5 years preferred

Knowledge, Skills and Abilities
-

1 The Director will ensure consistent, high-quality implementation of the complex care management program across assigned regions. They will monitor adherence to NCQA and internal standards, and support continuous readiness for audits and accreditation requirements, and lead the development and refinement of workflows, care models, and clinical pathways aligned with evidence-based best practices.

2. This role will be responsible for performance monitoring and improvement by tracking and reporting on KPIs, including readmissions, ED visits, inpatient admissions, cost of care, HEDIS, and patient experience outcomes, analyzing trends and outcomes to drive program enhancements and interventions, and using data to identify performance gaps and implement structured performance improvement plans.

3. Responsible for providing direct supervision and support two managers overseeing interdisciplinary care teams and overseeing and support for the Operations Improvement Associate. Foster a collaborative and high-performance work environment and provide guidance and professional development opportunities to team members.

5. Act as a key liaison to cross functional departments, collaborate with internal system wide and external stakeholders, to include community-based organizations and clinical partners to optimize care coordination. Responsible for providing clear and consistent communication with leadership and peers on program performance and strategy. When appropriate engage with patients and families to promote patient centered care.

6. Serves as a key liaison between complex care teams to include, Primary Care Providers, Practice Managers, Behavioral Health Providers, and Health plan care managers. Ensure regular two-way communication with Primary Care Providers to share patient updates, identifying key care coordination needs, and supporting practice level integration of the complex care model.

7. Represent the Complex Care program in relevant meetings, case conferences, and collaborative workgroups.

8. Identify opportunities for process improvements and efficiency enhancements that result in cost savings or cost avoidance. Ensure appropriate allocation and utilization of resources to meet program objectives.

9. Responsible for all managerial duties and tasks related to their assigned region, with supervision of 35-50 care team members (RNs, SW, CHW, Care Coordinators, Operations Associates).
