DESCRIPTION OF WORK

In this Aging Care Manager 2 position, you will serve as an Assessor by completing functional eligibility determinations and maintaining a caseload of vulnerable consumers in order to determine and ensure the appropriate level of care for each person. Work begins with completing training certifications, learning to use our assessment platform, and building your knowledge of the processes and resources available to us and our consumers. You will then receive and review referrals to confirm an assessment is needed; conduct interviews at home visits to obtain information from each consumer, members of the consumers' families, neighbors, and other persons necessary for the identification of social, economic, emotional, health, or physical problems for assisting the consumer; and complete required forms and documentation for submission to the appropriate parties while ensuring all aspects of your work are accomplished accurately and in a timely manner. Effective communication skills are essential for maintaining positive relationships with the individuals we serve, external agencies and facilities, and internal teams to promote a cohesive and cooperative working environment. - Full-time employment
- Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. May be required to be on a standby rotation for off-hours coverage.
- Telework: This position combines office and fieldwork. After the initial training and probationary period, telework may be considered on a case-by-case basis.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Westmoreland County. If no eligible candidates who live within Westmoreland County apply for this position, candidates who reside in other counties may be considered.
