Location Site: Salem Hospital Main Campus Dept: SH Psychiatric Patient Svcs Position Type: Full Time (40 Hours) Start time/ Shift/Schedule: 8:00 a.m. Your role with us: As an MSW Care Manager in Psychiatric Patient Services at Salem Health, you provide psychosocial support to adult psychiatric inpatients through assessments, care coordination, and discharge planning. You conduct timely evaluations, participate in multidisciplinary treatment planning, and collaborate with families and external resources to ensure safe, appropriate discharges. Your role also includes therapeutic group facilitation, mandated reporting, and maintaining thorough documentation in the EPIC EHR. What you'll do:
- Completes a comprehensive Psychosocial Assessment for each assigned patient within 72 hours of admission. Completes MMSE's per doctor's order in a timely manner.
- Documents social work interventions and patient contacts in the patient record in a clear, concise, and timely manner.
- Case coordinates the multidisciplinary treatment team to facilitate completion of the treatment plan. Consults with treatment team members, patient, and family/collaterals for coordination of interventions and services, including substance abuse services. Formulates comprehensive discharge plans specifically addressing patient/family dynamics, social/emotional needs and substance abuse needs. Liaisons with unit discharge planner to facilitate discharge planning activities of adult patients.
- Performs social work activities, including liaison with community providers, to assure timely discharge and activation of discharge resources coinciding with patients' clinical readiness for discharge. Facilitates patient/family understanding, decision making, and capacity to comply with medical, psychiatric, dual diagnosis and behavioral recommendations.
- Reports mandated child and elder abuse/neglect as required by Oregon law.
- Demonstrates competency in therapy skills utilizing professional group leadership standards. Consistently documents the assessment, planning, interventions and patient responses in a way that accurately reflects the patient's care and progress (e.g., Community Meeting, groups, activity attendance & participation, testing results as ordered, etc.) in the EPIC EHR.
Background/what we are looking for (MQ's)
- Master's Degree in Social Work from a CSWE accredited school required.
- LMSW, LCSW or CSWA (pursuing LCSW) required by State of Oregon.
- Required to complete SAVE of Managing Aggressive Training Equivalent within the first 30 days after hire and annual training thereafter.
- Ability to interact and communicate effectively and professionally with psychiatric patients, professional colleagues, insurance company representatives and the general public.
- Solid interpersonal communication skills, considerable emotional stability, good vocal/auditory acuity for communication, and ability to maintain confidentiality standards.
- Must have working knowledge of casework practices, including but not limited to: psychiatric assessment, diagnosis and treatment of adult and geriatric patients, family/group dynamics, case management and discharge planning.
