Responsibilities
  • Lead Quality Improvement Initiatives
  • Collaborate with clinical and operational leaders to implement data-driven quality improvement strategies using structured methodologies, focusing on infection control, clinical metrics, and patient experience.
  • Conduct Clinical Data Analysis
    Perform in-depth analysis of clinical and patient experience metrics to identify opportunities for improvement and guide strategic decision-making.
  • Ensure Clinical Documentation Accuracy and Compliance
    Conduct timely chart audits, review OASIS documentation, and provide education to staff on regulatory standards to ensure compliance with CMS Conditions of Participation, accreditation, and state licensing requirements.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses

Experience:

  • Home care and/or hospice case managment experience.

License(s)/Certification(s):

  • Current license to practice nursing in the applicable state for Registered Nurse
  • COS-C or HCS-O certification must be obtained within 12 monts of hire date.
  • Valid Mandatory Reporter course completion by state recruitment
  • Valid driver's license when driving any vehicle for work-related reasons
