Team Lead Job Position Summary:

The Team Leader, under the supervision of the Site Manager/Coordinator, is responsible for overseeing the program operations, administrative, program facility, and leadership role within the Community Residential Rehabilitation Services (CRRS) program. The Team Leader position works in the Community Residential Rehabilitation Services (CRRS) Program. CRRS, located in the Kensington section of North Philadelphia, are a homelike environment providing members with the maximum possible opportunity for independence and self-determination within a structured living situation. CRRS provides a range of educational activities, both inside and outside the residence.

The Team Leader will be responsible for the supervision and required case management of CRRS clients while in residence at the site. The CRRS program provides services that are individually based, community-focused, homelike, and recovery oriented with the primary goals:

  • Providing 24-hour supervision in a residential environment
  • Promoting stabilization of physical and behavioral health symptoms
  • Assisting individuals to develop and maintain skills necessary for living in the most integrated setting possible.
  • Promoting safety through assistance with self-preservation skills
  • Supervision of daily living activities
  • Providing medication management
  • Providing education training and support
  • Providing training in problem solving
  • Providing social and recreational skills training to improve leisure planning and to facilitate healthy and positive interpersonal relationships.
  • Providing supports that promote family inclusion.
  • Improving daily living skills
  • Attaining identified life, social or recreational skills, which are consistently addressed and reinforced.
  • Establishing positive interpersonal relationships with other members and counselors through weekly community meetings
  • Promoting healthy social interaction
  • Supporting individuals with partaking in day services in the community to further their social and emotional development.

Duties and Responsibilities:

  • Follow all agency procedures and assure compliance with all applicable regulations.?
  • Assume and maintain case management functions of assigned caseload.?
  • Supervise the daily schedule of residents and coordinate scheduling and staff coverage for all shifts.?
  • Maintain accurate and current service plans and progress notes.?
  • Organize and conduct individual, family and peer group counseling.?
  • Supervise clients in the use of community resources.?
  • Maintain program supplies.?
  • Provide crisis intervention for clients according to program policies.?
  • Be aware of client's financial liabilities, coordinate any financial changes with the billing department, and assist clients in securing third party resources when indicated.?
  • Be responsible for program petty cash.?
  • Drive agency vehicle for programmatic reasons?
  • Ensure that client medication is monitored in accordance with agency policy and procedures.?
  • Communicate all relevant client information and supervisory directives to all program staff.?
  • Be flexible with work schedule to meet needs of residents, program, and training.?
  • Ensure that all documentation is complete, current, utilizing electronic health record, and meets licensing requirements.?
  • Maintains regular monthly audits of all residential charts to ensure compliance per CRRS regulations.?
  • Keep case managers fully informed of any meetings or appointments.?
  • Assure that program site(s) are well maintained and clean.?
  • Assure an effective and appropriate Residential Services Plan has been developed for each client.?
  • Facilitate clients' rehabilitation in all areas, particularly those identified in the Residential Service Plan.
  • Interview and evaluate clients for entry into program in selection of applicants.?
  • Match clients with placements appropriate to ability and interest, via assessment and consultation.?
  • Act as liaison to other programs at COMHAR and outside of agency.
  • Provide on-call coverage as needed 24-7.
  • Provide training and supervision of residential counselors to ensure staffs are adequately equipped to maintain day to day program operations.?
  • Develops staff schedules to assure appropriate coverage and approves leave request in consultation with the Division Director of Intensive Residential and Community Services.
  • Completes performance evaluations and develops training/staff development plans with staff.
  • Monitor medications and supplements to residents.
  • Acts as liaison to COMHAR Facilities Department.
  • Be responsible for the maintenance of a facilities inventory of all equipment, furnishings, and utensils of the program.?
  • Supervise the orientation of all new residents and staff ensuring that all COMHAR procedures and applicable regulations are followed in this process.?
  • Maintain the facility in compliance with applicable federal, state, and local laws/standards.
  • Along with other team members, develops client-centered, recovery-oriented plans that foster the personal growth of the residents, reviews these plans on a regular basis and makes changes to these plans as the residents' needs and abilities change.
  • Directly provides and/or supervises staff in providing direct services to residents, including assistance with personal hygiene, tasks of daily living, educational, social, and leisure programs for which resident's input is elicited - including transportation in company vehicle if needed.
  • Directly provides and/or supervises staff in meal preparation, serving, and clean up as needed.
  • Maintains the residence & vehicle in a safe, clean and orderly state; does minor maintenance (i.e. change light bulbs, water plants, tighten loose screws, take out the trash/recycling, salt and/or shovel walk ways, turn off water at the source if there is a leak, etc.), informs Division Director of Intensive Residential and Community Services of needed repairs, secures contractors for needed repairs per corporate policy.
  • Interacts and cooperates with outside agencies for the betterment of the residents.
  • Transports residents to and from medical appointments, religious and/or cultural activities, and leisure activities as needed.
  • Immediately reports all resident crisis and/or unusual incidents to Division Director of Intensive Residential and Community Services.
  • Documents in residents' charts as required (including hand-written & EHR format).
  • Interviews, makes hiring recommendations, and assists with staff trainings.
  • Schedules coverage in the event of emergencies, staff call-offs and staff vacations.
  • Attends all meetings and training sessions as assigned.
  • Maintains petty cash and client funds per corporate policy.
  • Complies with all COMHAR Policies and Procedures with no unauthorized exception.
  • Directly provides and/or supervises crisis intervention as needed when providing direct care to residents.
  • Assigns & monitors staff trainings.
  • Approves supervisee's general progress notes in Evolv
  • Other duties reasonable and related to this position as directed by related supervisors.
Requirements

Requirement:

  • High School Diploma or GED equivalent with two (2) years of experience.
  • Residential and supervisory experience required for a minimum of 2 years.
  • Knowledge of the principles of psychiatric rehabilitation
  • Computer literacy and experience with electronic health records
  • Valid PA State Criminal, FBI (based on residency outside of PA), and Child Abuse clearances.
  • Clearances must be updated every 3 years per COMHAR policy.
  • CPR w/AED & First Aid certification required.
  • Valid driver's license required. (Must be able to move about COMHAR service areas throughout the day to provide appropriate oversight and coordination and meet job duties as needed and required.)
  • Physical examination including TB test six (6) months prior to employment and updated physical examination every two (2) years after employment.
  • Excellent demonstrable oral and written communication skills
