Nurse Practitioner Cardiology

Summary:
Reports to the Clinical Director of the Congestive Heart Failure Transition Clinic (CHFTC). With the approval of the attending physician of record and in collaboration with physicians case managers and nurses identifies CHF patients during their hospitalization who are at high risk for readmission and coordinates follow-up for evaluation in the CHFTC. Assists in the care and management of CHF patients in collaboration with the Primary Care Physician (PCP) to prevent readmissions.

Responsibilities:
During admission assists in the care and management of CHF patients including: abbreviated history and physical assessment medication reconciliation and intervention education of disease diet and lifestyle modification and coordination of care. Works with Care Management and other appropriate staff to assist and facilitate discharge. Schedules follow up in CHFTC for those patients who cannot be seen by their principal cardiologist or primary care physician within 72 hours of discharge.

During post-discharge/clinic visit evaluates for signs and symptoms of heart failure reviews current medications and reconciles the discharge summary and the continuity of care form. Provides opportunity for education and *Teach back*. Orders appropriate lab tests and adjusts medications based on clinical findings.

Reviews dietary and activity recommendations. Counsels patient and family members regarding the importance of daily weights low salt diet and medication compliance. Assesses cognitive function and understanding of treatment plan and level of family support. Coordinates instructions for care with Visiting Nurse Association (VNA) services. Ensures that follow up appointment with PCP or Cardiologist has been scheduled. Communicates with PCP/Cardiologist with a written case summary and/or with immediate verbal or email communication if more urgent action is required.

May perform other duties as requested by the Director of the CHFTC within the Nurse Practitioner scope of practice.

Assists in clinical research projects including data acquisition entry and analysis as needed.

Participates in quality improvement teams and other committees as needed.

Maintains quality assurance safety environmental and infection control in accordance with established hospital department policies procedures and objectives.

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

BASIC KNOWLEDGE:

Graduate of an accredited Nurse Practitioner program with MS degree. Certified by the American Nurses Credentialing Center (ANCC) or by the American Academy of Nurse Practitioners (AANP) to practice in the expanded role. License must include prescriptive privileges. Certification by the American Heart Association in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) is required.

EXPERIENCE:

Five years of experience with clinical cardiology patients preferably with previous experience with CHF patient populations. Cardiac NP subspecialty preferred.

Must have patient care services procedures and privileges approved by the sponsoring physician/Director.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Healthcare environment such as acute care hospitals physicians* offices and ancillary healthcare enterprises. Works with medical equipment and has direct patient contact. Any potential hazards are minimized by adherence to established safety procedures and precautions including use safety equipment and protective clothing.
