Job Summary:



The Program Manager of Care Management (TCM) & Population Health oversees the coordination and delivery of transitional care services to patients transitioning from one healthcare setting to another, such as from a hospital to a home or a skilled nursing facility, as well as oversight for compliance with value-based performance goals. They ensure a seamless continuum of care, reduce readmissions, improve patient outcomes, and enhance overall patient satisfaction by managing a multidisciplinary team and implementing best practices in transitional care, chronic care management, and population health metrics.

Key Responsibilities:



  • Develop and manage the transitional care management program to align with organizational goals and compliance standards.
  • Oversee the integration of care plans, ensuring effective transitions between healthcare settings.
  • Collaborate with healthcare providers, case managers, and discharge planners to ensure continuity of care.
  • Develop individualized care plans addressing patient needs, medication management, and follow-up care.
  • Ensure the program complies with applicable regulations, such as Medicare's TCM requirements, as well as additional payor contracted metrics.
  • Implement and track the success of quality improvement initiatives to enhance program effectiveness and patient outcomes.



Work Location:



Hybrid - this role is eligible for a hybrid schedule of 3 days per week on campus and as needed for in-person meetings.

Qualifications:



Minimum Qualifications:



  • Bachelor of Science in Nursing or related healthcare field.
  • 4 years of nursing experience.
  • 3 years of care coordination, case management, or transitional care services experience.
  • 1 year of supervisory/leadership experience.


A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year-for-year basis.

Applicants must meet minimum qualifications at the time of hire.

Conditions of Employment:



  • Current RN License in the State of Colorado
  • Current Basic Life Support (BLS) for Healthcare Providers issued by the American Heart Association or American Red Cross healthcare provider-level CPR certification
  • Must be able to work in person.
  • Must be able to travel to alternate locations as assigned
  • Complies with applicable CU Community Practice mandatory education and training.


Preferred Qualifications:



  • Master's degree in healthcare or another related field.
  • Expertise in database operations, including EMR (Epic) experience.
  • Public speaking or clinical education experience with adults and/or healthcare workers.
  • Experience with research and evidence-based platforms.

  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
  • Outstanding customer service skills.
  • Demonstrated commitment and leadership ability to advance diversity and inclusion.

Anticipated Pay Range:



The starting salary range (or hiring range) for this position has been established as $87,548 to $105,057.

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
