Our In-Home Care Team at Johns Hopkins Care at Home delivers, coordinates, teaches, and oversees nursing care for patients in their homes. Utilizing the nursing process, the team develops and implements care plans for those needing in-home health and support services. Beyond providing specialized treatments and procedures, the nurse educates patients and caregivers, conducts preventive health screenings, makes referrals, coordinates services within the agency and community, and advocates for patients to achieve health-related goals. The Registered Nurse In-Home Case Manager ensures that services are adequately provided, including scheduling visits based on the patient's identified needs and care priorities, all while adhering to Home Health Services policies and procedures. This role offers the unique opportunity to work across multiple locations in Washington DC and Montgomery County, ensuring a variety of work environments and patient interactions. The ideal candidate will be dedicated, adaptable, and committed to providing excellent patient care.
