PRIMARY CARE / MEDICAL COORDINATOR - LA CASA MHRC

As part of the Telecare family, La Casa Mental Health Rehabilitation Center (MHRC) is a 192-bed program providing assistance to adults 18 and older.

LA Basin Region includes: Los Angeles, Ventura, Kern County, Santa Barbara and AZ.

Shifts Available:



  • Full Time 1.00 | Day Shift | 8:00am-4:30pm | Monday-Friday
  • Expected starting minimum compensation: $21.00 Hourly


STATEMENT OF PURPOSE

We exist to help people with mental impairments realize their full potential.

MISSION STATEMENT

Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness.

POSITION OBJECTIVE

The Primary Care/Medical Coordinator assists the Primary Care Internist in coordinating medical treatment, including any necessary follow-up to clients at La Casa MHRC. The duties and responsibilities are performed under the directions and supervision of the facility Internist and the Director of Nursing.

QUALIFICATIONS

At least two years of working with clients who are severely and persistently mentally ill. Skills and abilities include organization, attention to detail and follow through, excellent communication skills and an awareness of third party payor requirements. Qualifications also include an understanding of HIPAA and the greater system of care.

QUALITY AND QUANTITY OF WORK



  • Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care.
  • Establishes and maintains a therapeutic relationship with residents.
  • Assists the Internist and Unit RNs in ongoing medical treatment and nursing care, including the following:

    • Assists in managing acute and chronic medical problems in consultation with the Internist and according to facility protocols.
    • Monitors client response to treatment.
    • Assists Unit RNs in accessing necessary care and treatment and arranging needed out-patient appointments.
    • Assists DON, ADON and Unit staff in processing laboratory results and any potential follow-up.


  • Assists the Unit staff with developing needed Care Plans.
  • If needed, assists with coordinating physical examinations of clients.
  • Documents pertinent client information as needed.


TEAM MEMBER PARTICIPATION



  • Participates as a team member and provides input via reporting observations, concerns and asking appropriate questions.
  • Assists in reviews and implements client treatment plans.
  • Responds to emergency/crisis situations.


ASSESSMENT



  • Observes, records and reports clients' behavior as per applicable State, County and Telecare guidelines.
  • Assists in assessing clients for acute and chronic medical problems

  • Constant sitting and standing 8 hours per day.
  • Frequent bending, stooping, twisting, reaching, turning, and squatting.
  • Occasional pushing 10-25 pounds for short distances. (15 to 20 feet)
  • Occasional pulling up to 10-25 pounds, possible wheel chairs.
  • Infrequent lifting 1-25 pounds from floor to overhead.
  • Occasional lifting 1-25 pounds from floor to waist.
  • Infrequent lifting 25-50 pounds from floor to waist.
  • Infrequent overhead reaching.
  • Constant Small motor and hand skills necessary to perform writing tasks and other simple grasping operations.
  • Occasional fine manipulation needed.
  • Occasional power grip and hand twisting required in both hands for clutching and grasping; to seize or hold firmly.
  • Normal or corrected near and far vision, in addition to non-inhibiting color vision. May read constantly for short durations throughout the day.
  • Constant talking and hearing throughout the work day.
