Job Title: LTSS Service Care Manager

Job Location: Member Facing - Assessments will take place via home visits in the Austin area


Job Duration: 4 Months (Potential to Extend)

Mon-Fri 8am-5pm


Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):

  • SC will have 3 days of visits and 2 days of documentation weekly
  • SC will have a caseload and work assigned tasks
  • SC will complete H2060 assessment
  • SC will fill put required forms for each assessment
  • SC will make follow up calls



Required Skills/Experience: Preferred Skills/ Experience:
1. 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions. 1. Service Coordination, Case Management or Nursing experience preferred.

Education Requirement: Bachelor's degree in Social Work, Health Services, Behavioral Science or related field, or equivalent experience. Software Skills Required: Microsoft Office, excel
